For all participants: Assume 1 admission and 1 discharge visit with the potential for 1 physician visit during follow-up. For those participating for shorter lengths of time (e.g., those with fewer chronic conditions and/or less complicated therapeutic regimes) this pattern will not be necessary or feasible. All other visits average 45 minutes. Telephone calls are estimated averages with duration of phone time estimated assuming contact with the patient/caregiver and no significant loss of time due to hold time, etc in referring for services, etc. * The total number of average visits for the time frame (e.g., a participant followed for 2 months received a total of 11 visits during the entire period). |
